Question: I, with 12 other older women in my church, received a Grey Paper from the Center for Medic are Services. We no longer automatic ally qualify for low-in- come subsidy as of January 1, 2008. What do we do?

Answer: You and the 12 other women can apply for extra help. You are encouraged to complete and return an enclosed lowincome subsidy application. A postage-paid, pre-addressed envelope is also included. Applying early is important so your extra help can be effective as early as January 1, 2008, if you qualify.

Question: If we do not complete the application where can we go to apply?

Answer: You can apply for extra help at the Social Security Administration or State Medical Assistance (Medicaid) Office at the Department of Family and Children Services.

Question: Who can we call for help with the application?

Answer: You may call your local GeorgiaCares Program at 1- 800-667-8387. They can assist you with the application.

Question: I received a letter in Orange Paper. Please explain the letter.

Answer: The letter in orange is CMS Publication No. 11199. This letter is for the beneficiary who continues to qualify for extra help in 2008 but will have a change in their co-payment level.

Question: What action do I have to take?

Answer: Keep the Notice. You do not have to do anything if information is correct.

Question: I received a letter from Social Security in September (Form 1026). It included a sheet with a summary of my income and resources. What action should I take?

Answer: You need to return the enclosed form within 30 days or extra help may terminate.
