Social Security Administration Review of Your Eligibility for Extra Help

QUESTION: I recently received a letter from the Social Security Administration advising me that my Extra Help may be terminated effective January 1, 2009. What do I do?

ANSWER: About 253,000 beneficiaries will receive this letter

beginning September 2, 2008. Beneficiaries must complete the enclosed form within 30 days and put it in the enclosed pre-paid envelope for processing or your Extra Help may end, effective January 1, 2009. QUESTION: I do not know how to complete the "Income and Resources Summary" form. Who can help me? ANSWER: If beneficiaries need assistance, they should contact the Social Security

Administration at 1-800-722-1213, their local Social Security field office, or call GeorgiaCares at 1-800-669-8387.
