QUESTION: I recently received a letter from the Social Security Administration advising me that my Extra Help may be terminated effective January 1, 2009. What do I do?

ANSWER: About 253,000 beneficiaries will receive this letter

beginning September 2, 2008. Beneficiaries must complete the enclosed form within 30 days and put it in the enclosed pre-paid envelope for processing or your Extra Help may end, effective January 1, 2009.
